Request for IRS Employee Identification Number

EIN numbers due June 16 for annual Form 990.
The Food Bank’s tax preparer has advised us that we need to obtain and
provide all Partner Agencies Employee Identification Numbers (EIN) for
our annual Form 990 to the IRS. This form lists all the agencies that we
serve, and we have not provided this imperative information before and
need to start doing so. Thank you for understanding.
Steps to provide EIN (and see Houston Food Bank’s form as an example)
Use a blank W-9 form provided here.
- Type or write on the document, completing items 1-7 (first section)
with your organization’s information (name of organization, address,
etc.)
- Include your EIN in the section labeled “Part I”
- When completed, please print (if you haven’t already) and sign and date the form (after Part II)
NOTE: Your organization may have a completed W-9 on file, if so, copy and submit that form
W-9 forms are due back to the food bank by June 16, 2017
Complete forms can be returned by:
- Scanning and e-mailing to agencyinfo@houstonfoodbank.org
- Faxing 832-202-2878
- Mail to Houston Food Bank at 535 Portwall St. Houston, TX 77029 (Attn: Agency Services)
