Core Connection update Core Connections update May partner e-newsletter Formation of this network of partner agencies and other social service providers is underway and growing. Core Connections Network will connect clients with resources and services at food pantries, food fairs and other places where people consistently go for food. In May, the first cohort of providers (all trained in Chronic Conditions Management) will learn to use the Efforts to Outcomes (ETO) software that has been developed to launch the project. By August, the second cohort will join them to reach the goal of twenty participants by year end. Houston Food Bank is taking the lead with the understanding that empowering its partners will empower their individual clients and families to take action toward improved health and well-being.
