Social Services Outreach: 832-369-9390 We can help Clients seeking assistance can access services in three ways:
Clients can call seeking information on pantry services in their local area, identify a local community partner agency, find where they can be assisted by a Social Services Outreach Coordinator for SNAP and other benefit enrollment. We can also provide referral information to other agencies that may be able to help with rental assistance and other social service needs. We have dedicated call center staff who also provide application assistance over the phone, or assist in mailing the client an application.
What do people need to bring with them? a) Identity (proof of who you are) - Current driver’s license or Department of Public Safety ID card. b) Immigration Status-Resident card (I-551), arrival/departure form (I-94) or papers from the U.S Citizenship and Immigration Services c) Proof of earned or unearned income. Award letter for Veteran benefits; workers compensation; unemployment; Social Security, Supplemental Security Income (SSI) or pension benefits. Loan agreement or statement from the person giving you money or paying bills (must show that person’s name, address, phone number and signature). d) Residence (proof you live in Texas) - Utility bill, drivers license or TX ID card, rent receipt, letter from landlord. e) Bank Accounts: Most current, if funds available exceed $1,000 or more. f) Rent or Mortgage costs: Recent checks; check stub or statement from the mortgage bank or landlord. Renters must provide the landlords name, address and phone number. g) Dependent care expenses: Receipts, signed stament from the person you pay, which shows when and how much paid. h) Child Support: Paid or received. Court papers that show what you must pay or district clerk record or letter from the parent who pays show how much, how often and the date usually paid (must have name, address, phone number and signature of the parent who pays). What services are offered on site? Clients can expect to receive full assistance in the completion of a state benefit application for SNAP (formerly known as Food Stamps) Medicaid, CHIP (Children’s Health Insurance Program) and TANF (Temporary Assistance for Needy Families). In addition to receiving assistance completing a benefit application form for SNAP, clients can also opt to receive an interview for SNAP benefits, and be issued and EBT (Electronic Benefit Transfer) Lone star card. This valuable service to clients ensures that should a household be deemed eligible for benefits, that the client is able to access those benefits immediately, once funds have been applied to card. This service is replicated at all of our 50 community partner sites with the Houston Food Bank's 535 Portwall location being the main “hub” and extension of what we do in the field as part of the outreach program.
