Recruiting new partners

In 2014, realizing that the closest thing to a magic wand may be our own ingenuity combined with internet technology, software, best practices and years of in-the-trenches experience, several partner agencies embarked on an integrated care project known as Core Connections. 

Eight members of the top-twenty-largest distributors of Houston Food Bank product with extensive experience and lofty goals have rolled up their sleeves to address some of the most complicated social problems faced by the Houston region.    

Recognizing that access to food can also be access to better nutrition, Core Connections builds on that premise while striving to orchestrate social services, healthcare and other community resources.  By learning from successful models, Core Connections teaches a step-by-step process of screening, triage, enrollment and chronic conditions management.  The Houston Food Bank is offering participants the opportunity to learn to use “Efforts to Outcomes” a software program adapted to gather data to report successful outcome measurements and progress.
